Sarah is not always the easiest person to get along with. She has been raised in wealth and privilege -- indeed, she was raised to accept it as her due -- and carries more than a little rich-girl hauteur. She enjoys the income and power that her position as a doctor offers her, and feels little compunction about using them for her own ends.
At the same time, though, Sarah has a strong sense of noblesse oblige. She genuinely feels that her responsibility as a doctor requires her to give any patient the best care possible, regardless of whether they can afford it or not. She is known for frequently working weekends at low-income clinics or urgent care facilities. This tendency has only increased since she was inducted into the Illuminati, and it's not unlikely that this is partly a compensating mechanism for the less savory things she's had to do under the Labyrinth's direction.
